2. Print and sign the Code of Conduct Agreement:

3. Then, print the following PDF file and complete it as your registration form:

   Registration Medical Release Form

4. Turn in checks, signed Code of Conduct Agreement, completed Medical Release Form, and a copy of your child's birth certificate (first time wrestlers only) by the first practice to:

Notes

• Participants need wrestling shoes. A shoe exchange is held at the first practice. All other gear (bags, singlets, headgear) is provided.

• Team sweats can be purchased.

• Registration fees are $100 for the first child and $80 for each additional sibling. Make checks payable to PAA.

• We may need to limit the overall size of the team or safety reasons since all wrestlers practice on the same mat. Therefore, priority will be given based on when the completed registration forms and checks are turned in.
